ASAH ABCD Future Care and Planning Survey

To ensure the foundation is in place for children to lead a good life as independently as possible as adults, planning is important for individuals and their families as well as for providers and the public to guarantee that funds are available and used wisely.

The annual ASAH ABCD Future Care and Planning Survey is an effort to assist ABCD agencies to anticipate and meet the future service system needs and desires of children as they enter adulthood.  ASAH distributes the survey to parents of students ages 15, 16, 17 and 18 in their member schools who work with their care manager and school faculty to provide confidential information about their child’s future and to help guide our agencies in their planning. We understand that there are many young people for whom this survey would be applicable who are not enrolled in ASAH schools, but we must start somewhere.

In addition, it is our hope that the survey provides an opportunity for families to begin or continue the conversation about their child’s future.

The first annual survey has been completed, and one of the most important data points is that that most families plan to have their child live at home or independently while those with multiple disabilities are considering group homes.

ASAH and ABCD are working to fine tune the survey including for what counties is their anticipated to be demand for group home placement.    Over time, we will be able to observe trends which will also be of value.
